Mobile Crisis

Elevate’s Mobile Crisis Response team is here for you 24/7/365. We provide onsite, in-person support to individuals experiencing a mental health crisis, whether that’s at your home or any other location.

Who can we help? Youth, adults, and families. Anyone, anywhere.

How to access services: Call or text our partners at 988 or Your Life Iowa if you are in crisis. Ask that a mobile crisis team be sent to you, and our team will call you when we are on the way.

Counties served: We offer mobile crisis services in over 20 counties across Iowa, assertive community treatment in SE Iowa and Black Hawk County, ISTART services across the State, behavioral health and substance use in Waterloo, with IRSH homes in 5 counties.

Crisis Care Intervention

Elevate’s Family and Youth Crisis Intervention Program provides in-home, strength-based programming to reduce the risk of continued or further involvement with the Department of Human Services, Juvenile Court Services, and/or Law Enforcement. The Family Crisis Specialist connects the child and family with the resources they need to move forward, while providing support and mediation services to help identify strengths, needs, and goals. Together with the family, the specialist develops and carries out action plans that support positive outcomes for everyone involved.

Referrals accepted from: Department of Human Services (DHS), Juvenile Court Services (JCS), and Law Enforcement (LE).

Medication Management Services

Within Elevate’s programming, we offer a range of services focused on medication management, basic blood lab monitoring, venipuncture, medication setup, health education on medication compliance, and more. Our health services team includes a psychiatric nurse practitioner and RNs. These services are included with our substance use services, Assertive Community Treatment, and IRSH services.

Medication Management. Our psychiatric mental health nurse practitioner provides psychiatric evaluation and treatment. This may include pharmacological management, psychotherapy, and social services based on your diagnosis and needs. You and your provider will work together to build a treatment plan centered around your strengths and goals.

Injection Administration. Licensed nurses or medical assistants administer injectable neuroleptics under the supervision of the treating psychiatric mental health nurse practitioner and/or physician.

Venipuncture. Certified staff at Elevate collect blood samples and send them to the lab for testing. This service is available at the Elevate clinic for your convenience.

GeneSight Testing. The GeneSight Test may be especially helpful for clients whose treatment resistance could be related to gene-drug interactions. This simple nasal swab test must be ordered by the psychiatric nurse practitioner. If you are interested, ask your provider at your next appointment to see if you qualify.

Substance Use Disorder Treatment

Strive Recovery is built for people who are ready to find the motivation within themselves to change the direction of their life. Our Substance Use Disorder Program provides structured, modern treatment for adults and adolescents.

Partial Hospitalization Program (PHP). This program runs daily for 12 days, after which you are assessed to transition to IOP or continue with additional time in PHP.

Intensive Outpatient Program (IOP): Mondays, Wednesdays, and Fridays for 3 hours.

Outpatient Level 1: Tuesdays and Thursdays for 2 hours.

Family Group Counseling and Meetings.

Individual Counseling Sessions included with programming and coupled with a Mental Health Therapist if needed.

Peer Support is available individually or in a group setting.

Elevate now offers OWI Evaluations. The fee for evaluation is $125.00.

I-START (Iowa Systemic, Therapeutic, Assessment, Resources, & Treatment)

I-START is a program that provides prevention and intervention services to people with intellectual and developmental disabilities (IDD) and mental health needs through crisis response, training, consultation, and outreach. Based on the nationally recognized START model, I-START is a team of professionals extensively trained in the mental health aspects of intellectual and developmental disabilities. The program provides community-based crisis prevention and intervention services to individuals with IDD and mental health needs, along with the people who support them.

I-START promotes systemic, person-centered approaches and provides outreach, training and education, clinical and systemic consultation, crisis plan development, 24/7 crisis support, and follow-up.

Who is appropriate for I-START Clinical Services? Individuals aged 17 or older with intellectual or developmental disabilities who are experiencing mental health or behavioral health needs. This includes individuals who may be high utilizers of crisis services, difficult to support in the community, or at risk of out-of-home placement or hospitalization.

Drop-In Center

The Drop-In Center is a welcoming space to spend time with people who understand what it’s like to face mental illness and substance use challenges. Everyone who has been touched by addiction and mental illness is welcome, including friends and family.
Adult Drop-In (18+): Mondays and Wednesdays 4:30–8:00 PM, Thursdays 1:00–8:00 PM, and the 1st and 3rd Sundays of the month 10:30 AM–2:00 PM.
Teen Support Group (ages 13–17): Tuesdays 4:00–7:00 PM.
Individual Peer Support Counseling is available upon request.

IRSH (Intensive Residential Support Home)

Elevate offers the IRSH program to adults diagnosed with a severe mental illness and intellectual disability. This program provides close, consistent support for individuals with challenging behaviors so they can feel like part of a community rather than living in a hospital or institutional setting.
Individuals in IRSH receive wraparound care including nursing services and may be part of the PPSS program or involved in IHH services. Elevate currently operates 5 IRSH homes across Iowa.

ACT (Assertive Community Treatment)

Elevate is currently accepting clients who qualify for ACT services in Southeast Iowa/Burlington Area and Black Hawk County.
Assertive Community Treatment (ACT) is an evidence-based program for individuals with serious and persistent mental illness. It is designed to support independent living and reduce hospitalization. Staff work alongside clients in their homes and throughout the community. ACT is less intensive than residential care but offers more hands-on support than regular supported community living.

Please note: ACT is a Medicaid-funded program only. You must have Medicaid to participate.

ACT Team includes: Program Manager, Case Manager, Nurse, Peer Support, CADC, Social Worker, and Prescriber.
